Renew patients transitioning to Bloom Health Studio

Excited to continue working you with you!! All patients following me to Bloom will receive 50% off of their new patient appointment visit through March 2027*.

Please follow these steps:  ​

-Create a portal account here

-Complete all new patient forms 

-Schedule a “new patient appointment”, since you are new to this practice

​

To transfer records, there are two options:

1) Fill out "Authorization to release medical records" on Renew portal (can leave address blank, use phone: (201) 839-3564, fax: (201) 275-1444)

2) Download all of your office notes & labs and upload onto this portal.

***If you would like to do labs before meet, first do records transfer and then message me on the patient portal. I'll review everything and then post an order for you***
